A plan’s month comes from one contract: the one with the largest disclosed commission on its most recent filing, preferring a medical contract when the plan has one. Most policies run to month-end and renew the next day, so a June policy year end is a July 1 renewal. The counts hold every plan we have a filing for, and filings arrive in different years — each row’s own policy year is stamped under the date it renews · sourced from U.S. DOL EFAST2 Form 5500.
