Form 5500 data inside Claude and ChatGPT

Ask Claude or ChatGPT who the broker of record is, which carrier writes a plan and when it renews. Answers come straight from the Form 5500 filings, with a link to the full record.

Connect it

Claude

  1. Press Add to Claude. Check that the address is www.form5500lookup.com, then press Continue.
  2. Under Authentication, choose Sign in when needed. Leave everything else as it is and press Add.
  3. In a chat, turn it on from the + button under Connectors.
  4. When Claude shows a Connect button, sign in with your Form5500Lookup email.

If you use Claude through your company, only a Claude owner may be able to add connectors. Send them this address.

ChatGPT

Form5500Lookup isn’t in the ChatGPT app directory yet. Until it is, you can add it yourself using Developer mode, which requires a paid ChatGPT plan.

  1. In ChatGPT, open Settings → Security and login and turn on Developer mode.
  2. Go to Plugins, press the plus button, name it Form5500Lookup, and paste this address as the connection:

  1. Under Authentication, choose Mixed. Lookups work straight away with no account.
  2. To use a subscription in the chat, ask ChatGPT to sign you in to Form5500Lookup, then sign in with your Form5500Lookup email.

Single State scoping

A Single State subscription is pinned to its state. A different state argument does not widen it; the response says so in its notes and in gate.scopedToState. With no state chosen yet, scoped tools return a link to choose it.

Matching a list from a script, and the feed files

The same matching the assistant does is available to a script at https://www.form5500lookup.com/api/lookup. It takes a bearer credential and up to 100 companies or EINs per call, and it accepts up to 2,000 companies a minute per credential. The credit rule is the same: one credit per plan record returned, nothing for a miss, and nothing for a record you already took this month. A name that could be more than one company comes back as up to three candidates, and you are charged only when you confirm one. Data Feed files can be fetched the same way, so a scheduled job on your side needs no one to sign in: https://www.form5500lookup.com/api/feed lists the weeks available, and https://www.form5500lookup.com/api/feed/latest/sponsors.csv.gz (or a specific week, and any of the five files) redirects to a download link that works for about a minute — follow the redirect. Each call checks that the licence is active and logs the delivery.

Rate limits and what your plan includes

Rate limits exist only to keep the service available: 30 requests a minute without an account, and 120 a minute when you are signed in. Nothing is capped by the day. What your plan counts is records, in credits. One credit buys one employer plan record, the first time you take it in a billing month, whether you download it or match it from your own list. Taking the same record again that month is free. A new Standard plan includes 25,000 records a month, and a new Single State plan includes 5,000 records a month in its state. Every plan writes at most 5,000 rows to a single file, as it always has. Your billing page shows what your plan includes and how many credits are left. If you need more, you can buy credits or the Data Feed at any time.

Keys for scripts

Only needed if you write your own scripts or run a scheduled job. Claude and ChatGPT connect without one. Create a key on your Connected Apps page. A key is shown once and stored only as a hash. It can do everything your plan can do, and that is re-checked on every call.
