Form5500Lookup is a service operated by Harper AI, Inc., a Delaware corporation (“Form5500Lookup,” “we,” “us”). By using this site you agree to these terms. Form5500Lookup lets you search and analyze public U.S. Department of Labor Form 5500 benefit-plan filings. You can find an employer’s benefits broker, insurance carrier, plan renewal timing, and disclosed commissions.
What this service is
Form5500Lookup is an information and market-intelligence tool. It is not fiduciary, investment, insurance, tax, or legal advice, and nothing here creates a professional relationship between you and us. The data comes from public records and may be out of date or incomplete; it is not a substitute for verifying directly with the U.S. DOL EFAST2 Form 5500. Always confirm against the official record before you act.
Not a consumer report (FCRA)
Form5500Lookup is not a consumer reporting agency and the data is not a “consumer report” under the Fair Credit Reporting Act (FCRA). You may not use Form5500Lookup, or any data obtained from it, in whole or in part as a factor in establishing a person’s eligibility for credit, employment, insurance, housing, or any other purpose regulated by the FCRA. Form5500Lookup is a sales-prospecting and research tool. It is not a screening service.
Accounts
Some features require an account, created by a passwordless email sign-in link. You are responsible for activity under your account and for keeping access to your email secure. Use the email associated with your subscription so billing and access stay linked.
Acceptable use
- No FCRA-regulated use, as described above.
- No scraping, bulk downloading, or automated access outside the features we provide. An AI assistant may read our pages for you, as you would, within the same daily page allowance as reading them yourself. Any other automated access is permitted only through our connector for AI assistants and the other machine-readable access we publish, under the Connector Terms.
- Do not extract our pages in bulk, copy them automatically, or resell what you take. Read a page, use what you find in your own business, and license the Data Feed if you want the whole set.
- No reselling or redistributing the data as a standalone dataset. One named exception: a Data Feed licensee may use the weekly file we deliver inside its own business, under the Data Feed licence set out in the Connector Terms. That licence permits the licensee’s internal use of the file and nothing more — no resale, no redistribution, no derivative dataset, and no sharing of the file or its download links outside the licensee.
- Records you take away. Your plan includes a set number of employer plan records each billing month, shown on your billing page. One credit buys one record the first time you take it in a month, whether you download it or match it from a list of your own, and taking the same record again in that month is free. Each file holds up to 5,000 rows. If you need more than your plan includes, you can buy credits or the Data Feed at any time.
- No use to harass, defraud, or unlawfully target any person or business.
- Respect rate limits and don’t attempt to disrupt or reverse-engineer the service.
Payment
Subscriptions are billed monthly through Stripe at the price shown on our pricing page. They renew automatically until cancelled and you can cancel anytime; see our Refund Policy for details.
Credits are a one-time purchase and never renew. You buy a pack, the credits are added to your account when the payment settles, and one credit is spent for each employer plan record you take — a downloaded row or a matched row — the first time you take it in a billing month. Taking the same record again in that month is free, and a record we cannot find costs nothing. Purchased credits do not expire; credits included with a paid subscription invoice are spent first and expire at the end of the period that granted them. Credits have no cash value, are not transferable, and end with the account. What a credit buys, and what happens at a zero balance, is set out in the Connector Terms.
The Data Feed — the whole dataset delivered as a weekly file — is sold monthly or annually. A monthly licence renews each month until cancelled. An annual licence is billed for the term, renews at the end of the term unless cancelled, and we send you a renewal notice before it does. Use of the file is governed by the Data Feed licence; refunds are covered by the Refund Policy.
Disclaimers & limitation of liability
The service and all data are provided “as is” and “as available,” without warranties of any kind, express or implied, including accuracy, completeness, merchantability, or fitness for a particular purpose. To the fullest extent permitted by law, Harper AI, Inc. will not be liable for any indirect, incidental, special, consequential, or punitive damages, and our total liability for any claim relating to the service will not exceed the amount you paid us in the twelve months before the claim.
Governing law & changes
These terms are governed by the laws of the State of California, without regard to conflict-of-law rules. We may update these terms; the effective date above marks the current version, and continued use after a change is acceptance. If you have a question, email [email protected].
