Form5500Lookup lets you search and analyze public U.S. Department of Labor Form 5500 benefit-plan filings — finding an employer’s benefits broker, insurance carrier, plan renewal timing, and disclosed commissions.
What this service is — and is not
Form5500Lookup is an information and market-intelligence tool. It is not fiduciary, investment, insurance, tax, or legal advice, and nothing here creates a professional relationship between you and us. The data comes from public records and may be out of date or incomplete; it is not a substitute for verifying directly with the U.S. DOL EFAST2 Form 5500. Always confirm against the official record before you act.
Not a consumer report (FCRA)
Form5500Lookup is not a consumer reporting agency and the data is not a “consumer report” under the Fair Credit Reporting Act (FCRA). You may not use Form5500Lookup, or any data obtained from it, in whole or in part as a factor in establishing a person’s eligibility for credit, employment, insurance, housing, or any other purpose regulated by the FCRA. Form5500Lookup is a sales-prospecting and research tool, not a screening service.
Accounts
Some features require an account, created by a passwordless email sign-in link. You are responsible for activity under your account and for keeping access to your email secure. Use the email associated with your subscription so billing and access stay linked.
Acceptable use
- No FCRA-regulated use, as described above.
- No scraping, bulk downloading, or automated access outside features we provide.
- No reselling or redistributing the data as a standalone dataset.
- No use to harass, defraud, or unlawfully target any person or business.
- Respect rate limits and don’t attempt to disrupt or reverse-engineer the service.
Subscriptions & payment
Paid features are billed monthly through Stripe at the price shown on our pricing page. Subscriptions renew automatically until cancelled and you can cancel anytime; see our Refund Policy for details.
Disclaimers & limitation of liability
The service and all data are provided “as is” and “as available,” without warranties of any kind, express or implied, including accuracy, completeness, merchantability, or fitness for a particular purpose. To the fullest extent permitted by law, Harper AI, Inc. will not be liable for any indirect, incidental, special, consequential, or punitive damages, and our total liability for any claim relating to the service will not exceed the amount you paid us in the twelve months before the claim.
Governing law & changes
These terms are governed by the laws of the State of California, without regard to conflict-of-law rules. We may update these terms; the effective date above marks the current version, and continued use after a change is acceptance. Questions: legal@form5500lookup.com.
